systems provide the opportunity to study failure points of an organizations
work process.
Capturing
near miss events, in addition to events that actually occur, provides valuable
data about recovery mechanisms and the causes underlying the events.
All event
reporting systems need a uniform method to describe events and a way to
discover the events root causes.
