A typical response to any type of event—whether near miss, no harm, or misadventure — is to ask, "Whose fault was it?" In other words, someone must be blamed—and punished.

The tendency to assign blame (to one or more individuals) is a natural, emotionally satisfying response to most disasters or accidents. However, blame and the ensuing punishment do little to improve the system and prevent future events.
